ActivHeal® Practice Nurse Survey – Terms and Conditions

This promotion is open to Practice Nurses, excluding employees of Advanced Medical Solutions Limited and its associated companies in the Advanced Medical Solutions Group and their immediate families, its agencies or anyone else professionally connected with the promotion.

To enter the promotion you must complete the Practice Nurse Survey.

Completing the survey will be deemed acceptance of these terms and conditions, and any other requirements set out in any relevant promotional material as if they were set out in these terms and conditions.

The promotion closes approximately four (4) weeks after the survey has been sent out.

The prize is fifty pounds sterling (£50.00) worth of Amazon e-vouchers (and/or higher module access to the ActivHeal® Academy modules and/or wound care guide), which may be subject to further terms & conditions as provided by Amazon. The value of the e-vouchers will automatically be converted to the user’s relevant currency by Amazon.

The winner will be contacted by HTML e-mail by Lumisi Ltd shortly after the promotion closes and the winner’s first initial and surname may be published on the ActivHeal® Academy website. Advanced Medical Solutions Limited’s decision in conjunction with Lumisi Ltd is final in all matters and no correspondence will be entered into.

This promotion cannot be used in conjunction with any other offer/promotion.

The winner may be asked to cooperate with Advanced Medical Solutions Limited for reasonable publicity purposes if so required and no payment will be offered.

Advanced Medical Solutions Limited has the right to disqualify any entry not made in accordance with these terms and conditions or which it has reason to believe has been made in bad faith. Only one entry per person/user is permitted.

In the event that Advanced Medical Solutions Limited (in conjunction with Lumisi Ltd) is unable to award the prize or it is unable to be received by the winning user, or Advanced Medical Solutions Limited (in conjunction with Lumisi Ltd) is otherwise unable to contact the winning user, Advanced Medical Solutions Limited (in conjunction with Lumisi Ltd) reserves the right to withdraw prize entitlement and award the prize to another user.

To the maximum extent permitted by law, Advanced Medical Solutions Limited shall not be liable for any loss and/or personal injury suffered by the winners as a result of their participation in the promotion.

Advanced Medical Solutions Limited reserves the right to withdraw, amend or temporarily suspend the promotion in its absolute discretion, provided that it shall not exercise this right unreasonably.

The promotion and these terms are governed by English law and subject to the exclusive jurisdiction of the English courts.
